What you can do

Insight that helps you act, not just report

Prioritise who to reach first

Build cohorts from record data, like a high HbA1c or no review in 12 months, and flag rising risk.

Reduce health inequalities

Filter outcomes and service use by ethnic group, locality, income, deprivation and condition, for one service or a whole area.

Show what services achieve

The outcomes manager reports uptake, engagement, survey responses and biomarkers, customised for council and ICB stakeholders.
