A plan in under two minutes

From the check to lower readings

Paul’s NHS Health Check finds his blood pressure high. His results come in from EMIS, Ben shares a heart health plan, and when his readings stay high, a walking group near him. Six Saturdays on, his readings at home are under his target.

Between appointments

Daily actions at home, readings back at the practice

A plan, trusted content and home readings of blood pressure, weight and cholesterol, shared back to your practice.

Daily actions at home, readings back at the practice

Daniel’s practice sees his blood pressure three times a year.

At home he takes a reading most days, and ticks off four small actions.

Each reading rises to its value, and his trend draws itself.

One Tuesday night: 168/102. His practice sees it that night, not at his review in October.

His medicines are reviewed. Week by week, his readings come down.

Everyone at risk on the list, a year of readings each. Only the few in orange need a review.

Small actions every day. Readings the practice can see.

Support more people at risk, without adding workload.

I think the app is a very useful tool – it has the right things on there to help and motivate you.
